How to Reduce No-Shows: A Playbook for Indian Appointments

How to reduce no-shows: take payment at booking, send a WhatsApp reminder 24 hours before, make rescheduling easy and publish a clear cancellation policy.

To reduce no-shows, take payment or a deposit when the slot is booked, send a WhatsApp reminder 24 hours before and a short nudge on the day, make rescheduling one tap, and publish a clear cancellation policy. Published research finds reminders alone cut missed appointments by about a third. Prepayment adds a commitment a reminder cannot.

This playbook is for Indian clinics, therapists, coaches, tutors and salons who lose money to empty slots. It covers what the research actually says, a reminder cadence you can copy, WhatsApp templates in English and Hinglish, and policy wording by profession. Every number below comes from a cited study or is a clearly labelled worked example.

What does a no-show actually cost you?

A no-show is a booked client who neither turns up nor cancels in time for you to refill the slot. The cost is simple to estimate: monthly loss = appointments per month × no-show rate × average fee. Use the no-show cost calculator to run your own numbers.

Worked exampleAppointments / monthNo-show rateAverage feeLost per monthLost per year
Physiotherapist, Pune12010%₹600₹7,200₹86,400
Therapist, Bengaluru (online)608%₹1,800₹8,640₹1,03,680
Home tutor, Jaipur (demo classes)4020%₹500₹4,000₹48,000
Salon, Hyderabad (bridal trials)3012%₹2,500₹9,000₹1,08,000

These rows are illustrations, not survey data. Plug in your own rate. Even a small practice with a 10% no-show rate usually loses more in a year than any booking tool costs. The time you spent preparing for a client who never came is extra and does not show up in the formula.

Do appointment reminders reduce no-shows? What the research says

Yes. Reminders are the best-studied fix for missed appointments. A systematic review by Hasvold and Wootton in the Journal of Telemedicine and Telecare pooled 29 studies of phone and SMS reminders and found reminders reduced non-attendance by about 34% of the baseline rate. Manual phone calls did a little better (about 39%) than automated SMS or calls (about 29%), and sending the reminder the day before worked as well as a week before.

A separate meta-analysis by Guy and colleagues in Health Services Research looked at 18 studies of SMS reminders. Across the 8 randomised trials, people who got an SMS reminder had 1.48 times the odds of attending. The timing of the message (24, 48 or 72+ hours before) made no significant difference.

There is Indian evidence too. A study at a dental college in Muradnagar, Ghaziabad, published in the International Dental Journal, sent SMS reminders to 96 outpatients and compared them with 110 who got none. On-time attendance was 79.2% with the reminder against 35.5% without. The authors called SMS reminders “an effective strategy to reduce nonattendance rates”. It is a small, single-centre study, so treat the size of the effect with caution.

StudySettingWhat it found
Hasvold & Wootton, 2011 (systematic review)29 hospital studies, mixed countriesReminders cut non-attendance by about 34% of baseline; manual calls about 39%, automated about 29%
Guy et al., 2012 (meta-analysis)18 studies, 8 randomised trialsSMS reminder: odds of attending 1.48 times higher; timing made no clear difference
Prasad & Anand, 2012Dental OPD, Ghaziabad, 206 patientsOn-time attendance 79.2% with SMS vs 35.5% without

The pattern is consistent: almost every study finds reminders help, and a personal touch helps a bit more. Most of these studies used SMS. WhatsApp is where Indian clients actually read messages today, so a WhatsApp reminder is the natural local version of the same intervention. Does taking advance payment reduce no-shows?

The evidence for deposits is thinner than for reminders, but it points the same way. A quality-improvement project at Ng Teng Fong General Hospital in Singapore (NUHS) added a deposit for sleep-study appointments, along with reminders and financial counselling. Monthly no-shows fell from 9.1% to 4.3%. It was a before-and-after project, not a randomised trial, so other changes may have helped.

The logic is easy to see in an Indian context. A client who has paid ₹1,500 through UPI has a reason to show up or to reschedule properly. A client who booked with a WhatsApp “ok done” has nothing at stake. Prepayment also changes who books: people who are not serious tend not to complete payment at all. This guide on advance payment for appointments shows how to set it up in India.

Book A Sloth does not yet have enough booking volume to publish its own no-show rates for prepaid versus unpaid bookings. We will publish aggregated numbers once the sample is large enough to be meaningful.

9 ways to reduce no-shows in your clinic or practice

  1. 01Take payment at booking. Full prepay for single sessions, or a deposit (25-50%) for long or high-value appointments. UPI makes this a 10-second step for the client.
  2. 02Confirm instantly. Send the confirmation the moment the slot is booked, with date, time in IST, address or meeting link, and the amount paid.
  3. 03Send a 24-hour reminder on WhatsApp. This is the reminder most studies tested. Include a one-tap reschedule link.
  4. 04Add a same-day nudge 2-3 hours before. Short and practical: time, place, parking or link.
  5. 05Make rescheduling easier than ghosting. If moving a slot takes one tap, clients move it instead of skipping it.
  6. 06Publish a cancellation and no-show policy. Put it on the booking page and in the confirmation so nobody is surprised.
  7. 07Keep the booking window short. A slot booked three weeks out is easier to forget than one booked for this week. Open your calendar 7-14 days ahead unless you have a reason not to.
  8. 08Call first-timers and high-value clients. The research suggests a personal call beats an automated message. Save calls for the bookings that matter most.
  9. 09Track repeat no-shows. After a second miss, ask that client to prepay in full before you hold a slot again.

How Book A Sloth helps reduce no-shows

Book A Sloth was built around the two fixes with the strongest case: payment at booking and reminders. Clients book from your page at bookasloth.com/yourname, pay up-front by UPI, card or netbanking through Razorpay, and the booking confirms on payment. Confirmations and a 24-hour reminder go out by email and WhatsApp, and clients can reschedule or cancel from the link instead of disappearing. See how booking notifications and reminders work.

The free Sloth plan charges 5% only on paid bookings; Sloth Active is ₹399 a month with 0% commission. Razorpay's gateway fee of about 2.4% applies on both. Details are on the pricing page.

Common questions

What is a normal no-show rate for appointments?

It varies widely by profession, city and whether clients prepay, so there is no single normal figure. Published studies report very different baselines. Track your own rate for a month: no-shows divided by total booked appointments. That baseline tells you how much reminders and prepayment are worth to you.

How far in advance should I send an appointment reminder?

Send the main reminder about 24 hours before, then a short nudge 2-3 hours before. A meta-analysis of SMS reminder studies found no significant difference between reminders sent 24, 48 or 72+ hours ahead, so the 24-hour reminder plus a same-day nudge is a sensible default.

Is WhatsApp better than SMS for appointment reminders in India?

Most of the published research tested SMS, and it works. In India, WhatsApp is where clients read and reply to messages, and it supports links, locations and quick replies. Use WhatsApp as the main channel and email as the backup with the receipt. Start with a clear appointment confirmation message so the reminder has something to point back to.

Should I charge a no-show fee?

The simplest version of a no-show fee is prepayment: the client pays when booking and the policy says a missed appointment without notice is not refunded. Chasing a fee after the fact rarely works. Keep the fee proportionate to your actual loss and state it clearly before the client pays.

How do I reduce no-shows without upsetting regular clients?

Offer a free reschedule up to a clear cut-off, such as 12 or 24 hours before, and make rescheduling one tap. Regulars rarely mind a policy that lets them move a slot easily. Reserve strict rules, like full prepayment, for clients who have missed twice. For the wording, use these cancellation and reschedule messages.
