There are three ways to manage appointments on WhatsApp: book manually in the free WhatsApp Business app, run an automated bot on the paid WhatsApp Business Platform (API), or share a booking link that takes payment and sends WhatsApp confirmations and reminders automatically. Manual works for a few bookings a day; beyond that, a booking link saves the most time.
This guide compares the three honestly, with Meta's current India rates for API messages, a worked example of the time manual booking takes, and a setup for clinics, salons, tutors and coaches who want to keep WhatsApp as the place clients talk to them.
Does WhatsApp have appointment booking?
The WhatsApp Business app gives you tools to talk to clients, not a booking calendar. Meta lists greeting messages, away messages, a business profile, a product showcase and instant replies among its features. These help you respond faster. The scheduling itself, checking your diary, holding a slot, collecting the fee and reminding the client, still sits with you or with another tool.
That is why most Indian professionals end up with one of the three setups below.
Option 1: Manual booking in the WhatsApp Business app
This is how most clinics, salons and tutors start. A client messages, you check your diary, offer two slots, confirm, share a UPI ID, wait for the screenshot, then remind them the day before. It costs nothing and feels personal. Saved appointment confirmation messages cut some of the typing.
- 01Set up a business profile with address, hours and a short service list.
- 02Save quick replies for your fee, address, UPI ID and available slots.
- 03Use labels such as New enquiry, Slot offered, Paid, Confirmed, Completed.
- 04Keep one calendar (Google Calendar or a diary) as the single source of truth.
- 05Set an away message for after hours and Sundays.
- 06Send reminders by hand the evening before.
The hidden cost is time. A typical booking takes 6-10 messages: enquiry, slot options, choice, payment details, screenshot, confirmation, reminder, and often a reschedule. Worked example: if each booking takes about 8 minutes of your attention across the day and you handle 60 bookings a month, that is 8 hours a month on scheduling chats. Two people messaging for the same slot at the same time is how double-bookings happen.
Option 2: A WhatsApp Business API bot
The WhatsApp Business Platform (often called the WhatsApp API) lets software send and receive messages on your business number. Vendors build booking bots on top of it: the client taps through a menu, picks a slot and gets automated reminders. It is powerful, but it has costs that vendor landing pages tend to bury.
Since 1 July 2025, Meta has charged on a per-message basis: a business pays when a message is delivered, and the rate depends on the message category and the recipient's country. In Meta's words, it “does not charge for messages from users to businesses”. Meta lists five categories: marketing, utility (such as confirmations and reminders), authentication (OTPs), service (free-form replies within 24 hours of a client's message) and Meta Business Agent, its own AI agent billed per token.
| Category (India, INR) | Typical use | Meta rate per delivered message |
|---|---|---|
| Marketing | Offers, festive promotions, broadcast | ₹0.8631 |
| Utility | Booking confirmation, reminder, receipt | ₹0.115 |
| Authentication | Login OTP | ₹0.115 |
| Service | Replies within 24 hours of the client's message | ₹0.115 after the first 1,000 each month per number |
Rates are from Meta's INR rate card effective 1 October 2026 and can change at the start of any quarter. From 1 October 2026, Meta gives each business number 1,000 free service messages a month and charges from the 1,001st. Check the live rate card before you budget.
Worked example: a clinic with 200 bookings a month sends a confirmation, a 24-hour reminder and a same-day reminder for each. That is 600 utility messages × ₹0.115 = about ₹69 a month in Meta charges. One festive offer to 500 past patients adds 500 × ₹0.8631 = about ₹432. Meta's fees are small. The bigger costs are elsewhere.
- The provider's own subscription or platform fee, which most API vendors charge on top of Meta's rates. Compare these carefully.
- Setup: a Meta business account, a verified number, and message templates written and categorised before you can send them.
- Bot design: menus that feel natural in English, Hindi or your regional language take time to get right.
- Payments and calendar: many bots still need a separate payment link and calendar integration, so you can end up paying for two tools.
An API bot makes most sense for a multi-doctor clinic or a large chain with steady volume and someone to manage it. For a solo professional, it is usually more machinery than the problem needs. If you are choosing a tool, compare these appointment booking apps in India.
Option 3: A booking link plus WhatsApp reminders
Here WhatsApp stays your conversation channel, but booking happens on a page. When a client asks for an appointment, you reply with one quick reply containing your booking link. They see open slots, pick one and pay by UPI. The system confirms the booking and sends WhatsApp reminders, so you only step in when a client needs a human. Deposit amounts and refund wording are in our guide to advance payment for appointments.

Manual WhatsApp vs API bot vs booking link: comparison
| Manual WhatsApp Business app | WhatsApp API bot | Booking link + WhatsApp reminders | |
|---|---|---|---|
| Setup | Minutes | Days to weeks, with a vendor | Under an hour |
| Monthly cost | ₹0 | Vendor fee + Meta per-message charges | Free plan or a fixed monthly fee |
| Your time per booking | High (6-10 messages) | Low once built | Low (one quick reply) |
| Double-booking risk | High at busy times | Low if synced to calendar | Low; one booking per slot |
| Payment at booking | Manual UPI + screenshot check | Often a separate link | Built into the booking step |
| Reminders | Sent by hand | Automated | Automated |
| Personal feel | Highest | Can feel robotic | Personal chat stays; admin is automated |
| Best for | Fewer than 3-4 bookings a day | Multi-staff clinics and chains | Solo and small practices |
Why reminders and prepayment matter for no-shows
Automating reminders is not only about saving time. A systematic review of 29 studies of phone and SMS reminders found they reduced missed appointments by about 34% of the baseline rate, and reminders sent the day before worked as well as earlier ones. Manual WhatsApp reminders work too, but they are the first thing that slips on a busy day. Our playbook on how to reduce no-shows covers the full reminder cadence and policy wording.
WhatsApp booking system for clinics and doctors: practical tips
- Keep patient conversations in one-to-one chats. Never discuss a patient in a staff WhatsApp group.
- Put timings and token or OPD rules in the confirmation, so patients do not arrive two hours early.
- Use the same booking link on your Google Business Profile, Instagram bio and visiting card.
- Keep one calendar connected to bookings, so walk-ins and online bookings cannot collide.
- Send prescriptions and reports only to the patient's own number, not to a relative's unless the patient asks.
How Book A Sloth fits a WhatsApp-first practice
Book A Sloth is the booking-link option. Clients book on your page at bookasloth.com/yourname with a phone OTP and no app download, pay up-front by UPI, card or netbanking through Razorpay, and the booking confirms on payment. Email and WhatsApp confirmations go out instantly, a 24-hour reminder is included on the free plan, clients can reschedule or cancel from the link, and two-way Google Calendar sync prevents double-booking. It is not a WhatsApp chatbot; you keep chatting with clients yourself.

Common questions
Can I manage appointments on WhatsApp for free?
Yes. The WhatsApp Business app lets you use quick replies, labels, greeting and away messages to handle bookings by hand. The cost is your time: each booking can take 6-10 messages, and reminders have to be sent manually. A free booking link with automatic WhatsApp reminders removes most of that work. Until then, WhatsApp appointment reminder templates save some typing.
How much does the WhatsApp Business API cost in India?
Meta charges per delivered message by category. On its INR rate card effective 1 October 2026, India rates are ₹0.8631 for marketing and ₹0.115 for utility, authentication and service messages, with 1,000 free service messages a month per number. Most API vendors add their own monthly fee on top.
Is online booking better than WhatsApp booking?
For more than a few bookings a day, yes. An online booking page shows only open slots, takes payment at the moment of booking and sends reminders automatically, which cuts double-bookings and chasing. Keep WhatsApp for conversation and share the booking link there.
Do I need a WhatsApp chatbot for my clinic?
Usually not for a solo doctor or small clinic. A quick reply with a booking link, automated confirmations and reminders covers most needs. A WhatsApp API bot makes more sense for multi-doctor clinics or chains with high volume and someone to maintain the bot and templates.
How do I stop double-booking when clients book on WhatsApp?
Use one calendar as the single source of truth and let clients pick from open slots on a booking page instead of negotiating times in chat. A booking tool that syncs with Google Calendar blocks a slot as soon as it is booked, so two clients cannot take it.
